Prostate Problems

Relevant specialty: Urology

As men age, the prostate often enlarges and can press on the urethra. Typical symptoms include a weakened urine stream, frequent urge to urinate, post-void dribbling, and nighttime urination. These complaints are usually benign and very treatable, but should be medically assessed. An examination provides clarity and can be approached tactfully and calmly.

Possible causes

01

Benign prostatic enlargement

The most common cause in older age: the tissue grows slowly and narrows the urethra, obstructing urine flow.

02

Irritation or inflammation of the prostate

An inflamed prostate can cause pain in the perineum, frequent urge to urinate, and burning, sometimes accompanied by feeling unwell.

03

Bladder function

If the bladder has to work against resistance for a long time, it may become overactive or weakened over time.

04

Age-related changes

Various age-related changes to the prostate are medically assessed to distinguish benign changes from those requiring further investigation.

Typical steps at the doctor's office

  1. 1During the consultation, your urinary symptoms will be assessed; a brief questionnaire helps to evaluate their severity.
  2. 2A physical examination and urine test are part of the basic assessment, supplemented by blood tests at the doctor's discretion.
  3. 3An ultrasound examination shows the prostate and bladder and assesses how well the bladder empties.

What you can do yourself

  • Drink fluids throughout the day and reduce the amount a few hours before bedtime.
  • Keep caffeine and alcohol moderate, as they can additionally stimulate the bladder and urge to urinate.
  • Take your time when urinating and empty your bladder as completely as possible without rushing.
  • Regular physical activity and pelvic floor exercises can support bladder function.
When a doctor's visit makes sense

If you have a strong urge but cannot pass water at all, this is urinary retention and an emergency: go to the emergency department immediately. If the symptoms when passing water increase, disturb your sleep, or a feeling of residual urine remains, further assessment is advisable. The general practitioner's office or urology will determine the cause and discuss the options.

Frequently asked questions

Does an enlarged prostate mean something serious?
Usually it is a benign enlargement that, while bothersome, is not dangerous. Medical assessment reliably distinguishes it from changes requiring further investigation.
At what age are such complaints common?
They increase from around mid-life onwards and are very common in older men. Even then, an examination is worthwhile to clarify the cause.
Can I do something myself?
Yes. Adjusted drinking habits, moderate consumption of caffeine and alcohol, and physical activity can relieve symptoms, but do not replace medical assessment.
